Briarfield Health Care Centers are unique in that the owners and managers of the centers play a very active role in the day-to-day operations of the facilities. This allows them to get to know each resident and their family members so that admission from the hospital or home is a successful transition for all parties. We take our responsibilities very seriously so if you have a concern or a comment, we want to hear it.

Comfortable accommodations, a soothing atmosphere and a rehabilitation plan that adjusts to meet your individual needs are the hallmarks of our services. Our holistic approach focuses on your goals and needs. Therapies are offered twice daily, seven days a week according to your needs, but are integrated throughout the daily routine of The Suites at Briarfield.

The rehabilitation team members employed by our centers include physical, occupational and speech therapists, who offer therapies such as strength and balance training, ambulation training, wound care, and pain management. Other therapies focus on Activities of Daily Living such as homemaking bathing, and dressing.

Under the direction of your physician our professional nursing staff is available for pain management, IV therapy, wound care or other specialized skilled nursing needs. Our nursing staff works closely with patients and families to make sure they understand medication management and wound care prior to discharge. The Dietician also provides education about nutrition, diet modifications and special diets prescribed by your physician.

A licensed social worker is available to discuss Advanced Directives such as a Living Will or Power of Attorney for Health Care. And, most importantly, social workers will arrange regular meetings with our team of experts so that you or your family member can be part of goal setting for your stay with us. Our discharge planners will also assist you with home care and equipment so that your return home will be successful and your recovery complete.

You should consider a short-term rehabilitation program if:

  • You have had elective surgery and your surgeon has set specific goals for your recovery
  • Have other medical conditions that are unstable or that may affect your recovery
  • Are not able to function at a level close to the level you functioned at before your hospitalization
  • You will return home alone
  • Have multiple levels in your home requiring you to climb steps

When choosing a short term rehabilitation unit consider the following:

  1. Does the program have a good reputation in the community?
  2. Are the physical therapists, occupational therapists and speech therapists employed on a full time basis, or are they contracted therapist that come and go?
  3. How often are the doctors in the building? Can you choose your own?
  4. What are the discharge statistics for the rehab unit?
  5. What patient and family teaching is available?
  6. How often are interdisciplinary team conferences held? Are you invited?
  7. Is the short-term rehab unit separated or integrated with other long term care residents?
  8. When you are touring a facility, does the "tour guide" seem knowledgeable and concerned with not only the prospective resident, but you and your feelings as well?
  9. Has the short-term rehab unit provided care to others with similar conditions?
  10. Does the short-term rehab unit provide weekly progress reports?
  11. Is there a Case Manager?
  12. Are families included in the discharge planning process?
  13. Is it in a convenient location for visiting?
  14. Is it a pleasant living environment?
  15. Be aware of the surroundings. Do the current residents appear content?
  16. Does the staff smile? Do they seem to enjoy their work?
  17. Does the short-term rehab unit have a social events calendar?
  18. Seek answers to your questions from an informed, knowledgeable source.
  19. Tour the facilities in your area before there is a crisis.
  20. Become aware of your finances and your insurance benefits.
  21. Have a healthcare proxy who knows your wishes.
  22. Investigate your options and pre-register for admission to a sub-acute rehab unit prior to having surgery.

Value and Service

At the time of admission, a personalized service plan will be developed based on a health assessment. From assistance with activities of daily living such as dressing and bathing to medication reminders, Renaissance will provide just the right level of care and support.
