BRIARFIELD AT THE RIDGE

In February of this year, the owners of Briarfield Health Care Centers purchased the former Glen View Manor in Mineral Ridge, Ohio in bankruptcy court. Briarfield Health Care Centers assumed management of the center on February 27, 2006 pending finalization of the sale. At the time of our entry into the center Glen View had been placed in Immediate Jeopardy status by the Ohio Department of Health following the second substandard survey in six months. Within 45 days of our entry the facility was re-surveyed and found to be in Substantial Compliance. This is the first time in over a year that the center has been in compliance.

The focus of our management team has been on improving quality of care and environment. In the initial 45-day period, in addition to bringing the facility back into compliance, the following steps were accomplished:

  • Immediate changes in upper management and other key positions. The current Director of Nursing has been employed by Briarfield Health Care Centers for more than five years as a Director of Nursing.
  • Implementation of systems, policies and procedures that have proven to be successful in our other centers
  • Centralizing all billing and accounts payable services
  • Converted nursing shifts from inefficient 8-hour shifts to more efficient 12-hour shifts resulting in improved retention, recruitment, and fewer call-offs due to the addition of an incentive program.
  • Quality Assurance systems put in place including: walking rounds, psychotropic medication review, nutritional assessments, fall-risk assessment and reduction programs, wound tracking and prevention, and weekly utilization review.
  • Ongoing equipment replacement program initiated
  • Repairs and improvements to the physical plant are ongoing with those items affecting resident care and quality of life receiving priority.

Following finalization of the sale in June, the remaining items were addressed:

  • Name change to Briarfield at The Ridge as part of a program to regain community confidence and to highlight a complete change in the philosophy of care and quality of care at the center.
  • J.P. Shah, MD assumed Medical Directorship in order to facilitate quality improvements in medical care and supervision.
  • Replacement of contracted therapy providers with in-house therapy staff.
  • A seasoned administrator already employed by Briarfield Health Care Centers has assumed leadership of the center.
  • The Physician Communication Procedure, Condition-Related Care Protocols, Symptom-Related Care Protocols and Patient Education Programs already in place at our other centers have been implemented

Briarfield at The Ridge is currently capable of providing the following services:

  • IV Therapies including specialty drips, morphine drips and TPN
  • Respiratory therapy including CPAP, Bi-PAP, and high liter flow oxygen
  • CAPD
  • Wound care including Wound Vac and the services of an enterostomal nurse
  • Psychological & Psychiatric services
  • Admissions after 6 pm, 7 days-a-week
  • Discharge Teaching
  • Home Safety Check by therapy prior to discharge
  • Therapies 6-7 days-a-week
  • On-site case management

And, in September, Briarfield at The Ridge successfully completed a second Ohio Department of Health survey.
